Several updated forms became required beginning January 1, 2026. Always confirm that you are using the most current OPS version before providing services or submitting licensing documentation.
2026 Requirements
OPS adopted updated administrative rules and revised several forms for use beginning January 1, 2026.
Continuing Education Hours Log

Open FormRenewing a Facilitator License
OPS facilitator licenses have a one-year term. The TLC renewal window opens 120 days before expiration, and OPS advises submitting a complete renewal application at least 60 days before the license expires.
120 days
Renewal window opens in TLC.
60+ days
Submit a complete application for timely-renewal status.
Expiration date
An untimely renewal may prevent continued operation if processing is incomplete.
Continuing Facilitator Education
Beginning January 1, 2026, licensed facilitators must report at least four qualifying continuing education hours completed during the annual license period preceding renewal.
Minimum: 4 hours
Include the signed log and supporting completion documentation with the renewal application.
